Occupational Therapist

Concord Post Acute is seeking a skilled Occupational Therapist to join our team.

We have multiple openings available for full-time, part-time, and per diem schedules.

Key Responsibilities


* Develop and execute personalized treatment plans based on physician orders.


* Provide direct patient care to improve daily living skills and independence.


* Document patient progress, evaluations, and certifications in accordance with all regulatory requirements.


* Collaborate with the interdisciplinary team on patient care, progress, and discharge planning.


* Supervise Occupational Therapy Assistants and train Restorative Aides.


* Assess and facilitate ordering of necessary durable medical equipment.

Requirements


* Current and valid Occupational Therapy License.
