Key Responsibilities:


* Provide direct patient care under the supervision of an RN


* Administer medications and treatments as prescribed


* Monitor and document patient conditions


* Ensure patient safety and maintain high standards of care


* Assist with patient assessments and coordinate care with the interdisciplinary team

Qualifications:


* Current California LVN License


* CPR Certification (required)


* Previous experience in a post-acute or skilled nursing facility (preferred)


* Strong communication and patient care skills


* Ability to work independently and as part of a team
