Vineyard Post Acute is seeking Occupational Therapists (OTs) to join our dedicated rehabilitation team.

Job Responsibilities


* Evaluate and develop individualized treatment plans for residents


* Implement evidence-based therapy interventions to improve daily living skills


* Work collaboratively with an interdisciplinary team to enhance resident outcomes


* Educate residents and families on adaptive techniques and rehabilitation goals


* Maintain accurate documentation in compliance with regulatory standards

Qualifications


* Active Occupational Therapist (OT) license in California


* Passion for helping patients regain function and independence


* Strong teamwork and communication skills


* SNF experience preferred, but new graduates are encouraged to apply
