Job Title: Licensed Vocational Nurse (LVN) -Part-Time
Location: Visalia Post Acute - Visalia, CA
Shifts: AM or NOC | 12-Hour Shifts
Starting Pay: $30/hour
Employment Type: Part-Time

We're seeking compassionate, dependable, and licensed LVNs to join our dedicated team for AM and NOC 12-hour shifts.

Key Responsibilities

Clinical Care & Supervision:


* Direct and monitor the day-to-day functions of nursing assistants


* Ensure quality care in accordance with resident care plans


* Admit, transfer, and discharge residents as needed


* Administer medications and treatments as prescribed


* Monitor and report changes in residents' conditions

Documentation & Reporting:


* Accurately complete and maintain residents' medical records


* Record physician orders, chart treatments, and update care plans


* Prepare required forms, evaluations, and reports

Leadership & Communication:


* Provide guidance and oversight to CNA staff


* Conduct rounds to ensure quality of care and staff compliance


* Communicate with residents, families, physicians, and interdisciplinary teams


* Participate in staff meetings, in-services, and training sessions

Compliance & Safety:


* Ensure adherence to infection control, safety, and facility protocols


* Participate in audits, surveys, and quality assurance programs


* Maintain confidentiality of resident and facility information

Qualifications


* Current and valid California LVN license


* CPR certification required


* Graduate of an accredited LVN program


* Knowledge of Medicare/Medi-Cal regulations preferred


* Experience with PointClickCare (PCC) preferred


* Ability to work independently and manage multiple priorities


* Strong written and verbal communication skills


* Must be able to lift up to 25 lbs and perform physical job duties as required
