Occupational Therapist (OT)
Location: Westgate Gardens Care Center - Visalia, CA
Status: Full-Time / Part-Time / Per Diem

Westgate Gardens Care Center is currently hiring a skilled and compassionate Occupational Therapist to join our rehabilitation team.

We are a premier skilled nursing facility focused on restoring quality of life for our residents through innovative and personalized therapy services.
Key Responsibilities:


* Evaluate residents within 24 hours of physician referral; document any delays clearly.


* Develop individualized, evidence-based treatment plans with physician approval.


* Deliver direct OT services and support patients in regaining independence in daily living.


* Supervise and collaborate with Certified Occupational Therapy Assistants (COTAs).


* Provide training for staff on therapeutic approaches and restorative care.


* Maintain accurate documentation including evaluations, daily notes, and progress updates.


* Assist with discharge planning, including caregiver education and durable medical equipment recommendations.


* Ensure a safe and therapeutic environment by maintaining proper equipment and practices.

Qualifications:


* Bachelor's, Master's, or Doctorate degree in Occupational Therapy.


* Active OT license in the State of California (in good standing).


* Experience with documentation systems such as Casamba or PointClickCare preferred.


* Excellent communication, organizational, and interpersonal skills.


* Ability to manage physical demands including lifting and supporting residents.
