Pay Rate: $38 - $42 per hour (DOE)
Schedule: Full-Time or Part-Time

Willow Creek Healthcare Center is seeking a skilled and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team.

We are a skilled nursing facility committed to providing excellent patient-centered care and fostering a supportive work environment for our staff.

Key Responsibilities:


* Deliver physical therapy treatments as directed by the supervising Physical Therapist


* Accurately record daily treatment notes and weekly progress reports per PT Board standards


* Assist in department upkeep and equipment maintenance


* Participate in care planning and interdisciplinary rehab conferences


* Collaborate with nursing staff to train Restorative Aides


* Communicate effectively with healthcare team members regarding patient care plans and progress


* Conduct in-service training for facility staff as needed


* Educate patients, families, and caregivers in preparation for discharge


* Ensure documentation meets all facility and regulatory requirements

Qualifications:


* Licensed as a Physical Therapist Assistant in the state of California


* Experience with Casamba and PointClickCare preferred


* Strong written and verbal communication skills


* Knowledge of therapy best practices and documentation standards


* Must maintain continuing education per state board guidelines


* Ability to lift/push/pull up to 100 lbs occasionally and meet physical demands of the role


* Ability to relate positively and professionally with patients, families, and team members
