What You'll Do:


* Evaluate and treat post-acute patients with individualized care plans


* Collaborate with other therapy professionals, nursing staff, and families


* Promote functional recovery, independence, and mobility


* Document progress and outcomes according to facility standards

Requirements:


* CA-licensed Physical Therapist (or eligible)


* SNF experience a plus, but new grads welcome!
