We are currently seeking a caring, organized, and driven Social Services Assistant to join our interdisciplinary team.

Position Summary:
The Social Services Assistant supports the Social Services Director in helping residents and their families adjust to life in the facility.

The ideal candidate will demonstrate strong interpersonal skills, attention to detail, and a genuine passion for serving the elderly population.

Key Responsibilities:


* Assist with psychosocial assessments and the development of individualized care plans.


* Support residents and families during admissions, transfers, and discharges.


* Coordinate services with community agencies and help arrange outside appointments and transportation when necessary.


* Document progress notes and maintain accurate social services records in accordance with facility policies and state/federal regulations.


* Participate in care plan meetings and collaborate with the interdisciplinary team.


* Provide emotional support to residents and families, including crisis intervention when needed.


* Promote residents' rights and advocate for quality care and services.

Qualifications:


* High school diploma or equivalent required; Associate's or Bachelor's Degree in Social Work, Psychology, or related field preferred.


* Experience in a long-term care or healthcare setting strongly preferred.


* Excellent communication and documentation skills.


* Strong organizational and time management abilities.


* Compassionate, patient, and able to handle sensitive situations with professionalism.
