Linwood Meadows Care Center is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our dedicated rehab team.

Key Responsibilities:


* Provide occupational therapy treatments under the direction of a licensed Occupational Therapist (OT)


* Deliver therapeutic interventions to help residents achieve functional goals


* Document daily treatments, progress notes, and updates in compliance with state and facility guidelines


* Assist in care planning and discharge preparation


* Educate residents and caregivers on techniques for improved mobility and daily living activities


* Collaborate with interdisciplinary team members including nursing, PT, and SLP


* Maintain a clean and safe treatment environment


* Participate in in-services, trainings, and facility quality improvement initiatives Qualifications:


* Active California COTA license in good standing


* Graduate of an accredited Occupational Therapy Assistant program


* CPR certification (or ability to obtain upon hire)


* Experience in skilled nursing or geriatric rehab preferred


* Strong communication, time management, and documentation skills


* EMR proficiency (experience with Casamba or similar systems a plus)
