Resident Care Manager (RN or LPN)
Position Summary:
The Resident Care Manager is responsible for overseeing the delivery of care to residents within the skilled nursing facility.
This role ensures that the highest standards of care are provided, focusing on resident well-being, staff coordination, and compliance with regulatory requirements.
The Resident Care Manager will collaborate with interdisciplinary teams to develop, implement, and monitor care plans, provide supervision to nursing staff, and maintain an optimal environment for residents' health and safety.
Key Responsibilities:
* Resident Care & Assessment:
* Oversee the development, implementation, and evaluation of individualized care plans for each resident based on their specific needs and medical conditions.
* Conduct regular assessments of resident health status, monitor vital signs, and document changes in condition.
* Ensure the effective management of residents with chronic illnesses, post-surgical recovery, or those requiring specialized care.
* Team Collaboration:
* Lead and coordinate the interdisciplinary team, including nursing, therapy, dietetics, social services, and other healthcare professionals.
* Supervise and mentor nursing staff, including LPNs and CNAs, to ensure optimal delivery of care.
* Facilitate family meetings to discuss care plans, progress, and any concerns related to resident care.
* Compliance & Documentation:
* Ensure that all care procedures adhere to regulatory guidelines (state and federal), including the Oregon Nurse Practice Act, and SNF policies.
* Maintain accurate and up-to-date medical records in compliance with HIPAA and facility policies.
* Regularly review and audit documentation for accuracy, completeness, and adherence to standards.
* Resident Advocacy & Communication:
* Serve as the primary advocate for residents, ensuring their needs and preferences are communicated and addressed.
* Build positive relationships with residents and their families to enhance satisfaction and trust in the care provided.
* Ensure that any concerns or grievances from residents or families are promptly addressed and resolved.
* Staff Development & Training:
* Provide ongoing education and training for nursing staff on best practices in resident care, safety protocols, and regulatory requirements.
* Assist in staff performance evaluations and provide feedback to promote professional growth.
* Quality Improvement:
* Participate in quality improvement initiatives to enhance resident care and operational efficiency.
* Analyze data from incident reports, resident surveys, and other sources to recommend improvements in care delivery.
Qualifications:
* Education:
* Current LPN or RN license in the state of Oregon
