Job Requirements:


* Active/Valid (Unrestricted) Indiana LPN License


* 1 full year of Long Term Care experience within the last 3 years


* Physical within the last 6 months that states you are "free from" or "without evidence of" communicable diseases


* Current PPD or Chest X-ray (we can do the PPD at no cost to you)


* 2 Professional References (RN's or LPN's)


* Clear Criminal History Report (we will obtain at no cost to you)
