UW Health in northern Illinois iis seeking a dedicated and compassionate Internal Medicine Advanced Practice Provider (ACNP, AGANP, or PA-C) to join our primary care team.
As the preferred primary care provider in Northern Illinois, UW Health offers a patient-centered approach to care, focusing on comprehensive health and wellness for individuals across all stages of life.
This position provides an exciting opportunity to work within a collaborative, dynamic environment, serving our diverse patient population.
Your clinical support includes the following:
* Providing primary care services for a wide range of adult patients with both acute and chronic conditions.
* Collaborating with a multidisciplinary team that includes physicians, nurses, care coordinators, and specialists to provide holistic care.
* Managing patient care, conducting physical exams, performing diagnostic tests, and interpreting results.
* Educating patients and families on managing chronic conditions and promoting preventive care.
* Participating in patient care planning and follow-up to ensure continuity of care.
Qualifications:

Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation).
