As a licensed vocational nurse care manager, you will play a vital role in coordinating care for youth with complex medical and behavioral health needs.
The most qualified candidate will have experience or familiarity with behavioral health and substance use disorders.
Have strong knowledge of medical terminology and care coordination for youth services.
Incumbents must have the ability to work independently in a field-based role, traveling to members' homes and other various locations throughout Riverside County.
