The Licensed Vocational Nurse (LVN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs).
Care is delivered in accordance with current federal, state, and local regulations, facility policies, and under the direction of the Director of Nursing (DON) or Nurse Supervisor to ensure the highest quality of resident care.
Essential Duties & Responsibilities
* Provide direct nursing care in accordance with resident assessments and care plans
* Supervise and support CNAs to ensure quality care delivery
* Administer medications and treatments as ordered by physicians
* Accurately document nursing care, medication administration, and resident status
* Monitor residents for changes in condition and report findings promptly
* Participate in admissions, transfers, and discharges as assigned
* Ensure compliance with infection control, safety, and sanitation standards
* Communicate effectively with residents, families, physicians, and interdisciplinary team members
* Assist with restorative and rehabilitative nursing programs
* Respond appropriately to emergencies and resident needs
* Maintain resident confidentiality and comply with HIPAA regulations
Supervisory Responsibilities
* Provides direct supervision to CNAs during assigned shift
* Ensures staff adhere to policies, procedures, and scope of practice
Qualifications
Education & Experience
* Graduate of an approved LVN/LPN program
* Long-term care or skilled nursing experience preferred
Licenses & Certifications
* Current, active, and unencumbered LVN license in the State of California
* Current CPR certification
* Knowledge of PCC preferred

General Purpose
The primary purpose of the Activity Assistant is to assist the Activity Director run a creative and interactive activities program.
This must be done in accordance with current federal, state, local and corporate standards, regulations, and guidelines to assure that an on-going program of activities is designed to meet, in accordance with the comprehensive assessment, the interests and physical, mental, and psychosocial needs of each resident.
Essential Duties
* Participate in planning and conducting of individual, small and large group activities.
