General Purpose
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by
your supervisors.
Essential Duties
• Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary.
• Use only authorized abbreviations established by this facility when recording information.
• Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
• Report all accidents and incidents you observe on the shift that they occur.
• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Report any known or suspected unauthorized attempt to access facility's information system.
• Perform only those nursing care procedures that you have been trained to do.
• Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).
• Greet residents and escort them to their room.
• Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate.
• Make resident comfortable.
• Inventory and mark the resident's personal possessions as instructed.
• Store resident's clothing.
• Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.
• Transport residents to new rooms or to the receiving area.
• Assist with loading/unloading residents' to/from vehicles as necessary.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
• Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.
• Report all complaints and grievances made by the resident.
• Participate in and receive the nursing report as instructed.
• Follow established policies concerning exposure to blood/body fluids.
• Make beds as instructed.
• Put extra covers on beds as requested.
• Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed.

The Behavioral Health Licensed Practical Nurse (LPN) will function as a member of the Integrated Health Care Behavioral Health Prescriber Services Nursing Team. The LPN will assist the Behavioral Health Psychiatric Prescribers in their provision of care while providing a full range of nursing and medical support services for clients enrolled in IHC’s Article 31 and Article 32 clinics.
License Practical Nurse (LPN) Job Responsibilities:
* Manage the Nursing Triage and Medication Line; responds to patient triage and support calls, coordinates care as needed.
* Coordinate the Prescriber Services Bridge Script Process. Assists in providing medication refills, prior authorizations, and tasks related to ensuring prescribed medication regimes are made available to clients per agency and program policies.
* Manage Initial Psychiatric Evaluation Process; inclusive of coordinating receipt of records, blood work and EKG results, and necessary clinical information from the client’s care team.
* Efficiently utilize the electronic health record, referral platforms, and other IT resources to facilitate efficient management of administrative functions. Maintain detailed documentation, spreadsheets, and workflows to effectively complete job duties in an organized and detail-oriented manner.
* Administer Long-Acting Injection Medications, when necessary due to RN absence, when ordered by a clinic prescriber and uses best practices as defined in the Nursing standards.
* Facilitate Health Monitoring to address health implications and support recovery, when necessary due to RN absence.

General Purpose
The Staff Physical Therapist Assistant assists the Staff Therapist with patient related activities and direct patient care.
Essential Duties
• Treat patients as directed by Physical Therapist.
• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department.
• Participate in Patient Care and Rehabilitation Conferences, as needed.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in in-services training program for other staff in the facility.
• Record treatment changes per policy and procedures.
• Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Report any problems with department equipment so that it is maintained in good working order.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Qualification
Education and/or Experience
Licensed as a Staff Physical Therapy Assistant Board of the States.
Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care.
Language Skills
Ability to read technical procedures.
Ability to read and comprehend policy and procedure manuals.
Ability to effectively present information and respond to questions from managers, coworkers and families.
Mathematical Skills
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Reasoning Ability
Ability to solve practical problems.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registrations
Licensed as a Physical Therapist Assistant in the state.
Employee must meet continue education requirements per state practices.
Must maintain a license in good standing at all time with the state board.

General Purpose
The Staff Physical Therapist evaluates and treats patients, communicates with families, physicians and other health team members and maintains documentation of services in the medical records.
Conducts in-services and training for facility staff on an ongoing basis.
Essential Duties
• Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral.
If unable to complete evaluation in 24 hours, documentation in medical record clearly defines cause for delay.
• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Supervise Physical Therapy Assistants in direct patient care and patient related activities, following state practice act.

• Recommend and facilitate the ordering of necessary durable medical equipment for patients to facilitate independence in mobility; i.e., braces, walkers, canes, elevated toilet seats and grab bars.
• Report any problems with department equipment so that it is maintained in good working order.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.
Supervisory Requirements
Supervises PT assistants, aides and students.
Qualification
Education and/or Experience
Bachelor's degree in Physical Therapy, prefer graduates of Masters or Doctorate Program in Physical Therapy.
