Hospice Registered Nurse Looking for a motivated, compassionate nurse to provide exceptional care to our patients.
Position: 32 -40 hour position Primarily 8:00 a.m.
to 4:30 p.
m Monday - Friday with call every 8th day (includes occasional weekends) Responsibilities: Observes, collects, and records objective/subjective data on admission assessment and initiates plan of care.
Completes required patient visits to assess, collect, and accurately document Hospice Outcome & Patient Education (HOPE) data in accordance with CMS regulations and hospice policy.
Coordinates care between healthcare providers and hospice interdisciplinary group.
Ensures the plan of care is individualized, accurate and appropriate for the needs of the patient.
Revises the plan of care as needed and re-evaluates the patient's health care needs regularly.
Observes and anticipates potential signs and symptoms of advancing illness and initiates symptom management interventions proactively in collaboration with the physician and/or the interdisciplinary group (IDG) and the patient and family.
Documents skilled nursing visits at point of care and other appropriate clinical notes on the electronic medical record (EMR).
Participates and provides required documentation for IDG meetings and care conferences.
Plans and counsels with the family and provider for care which is feasible within the physical, financial and emotional resources of the family and within the philosophy of care under hospice.
Educates the patient and family on diagnoses, disease process, disease progression, and treatments ordered by the provider.
Performs diagnostic tests as ordered and obtains laboratory specimens when indicated.
Educates patient/family/caregiver on findings of tests.
Gives preventive immunization or treatments under medical direction.
Shares in procedures designed to prevent and control the spread of communicable diseases and applies measures for prevention and control as required by local health regulations.
Applies fees associated with the hospice program and services and uses cost containing measures in delivering care.
Ensures that charges are applied appropriately.
Supervises the home health aide by making a supervisory visit to the patient's residence at least every 14 days with or without the aide present in homes with aide services.
Provides in-services to staff, assists with orientation of hospice personnel and volunteers, including training of hospice aide skills and observing competence of aide skills.
Makes annual on-site visits to where a patient is receiving care to observe and assess aide while performing care.

Under the direct supervision of the Clinical Research Director, the Mobile Medical Van Driver & Outreach Assistant will safely transport and operate a mobile medical van to community locations throughout San Francisco while assisting clinical staff with outreach, participant engagement, and clinic flow.
The Driver will also check on the status of the van regularly and ensure all parts and components are functioning and vehicle maintenance is up to date.

Essential Duties
* Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
* Report all changes in the residents' condition to the Health and Wellness Director as soon as practical.
* Record all entries on notes, SPAs, and incident reports , etc., in an informative, descriptive manner.
* Report on all accidents and incidents you observe on the shift that they occur to the Executive Director and Health and Wellness Director .
* Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Executive Director .
* Report any known or suspected unauthorized attempt to access communit y's information system.
* Perform only those care tasks that you have been trained to do.
* Ensure that the resident's room is ready for the resident (i.e., bed ma de ).
* Greet residents and escort them to their room.
* Introduce residents to his/her roommate, if any, and other residents and personnel as appropriate.
* Make residents comfortable.
* Inventory and mark the residents' personal possessions as instructed.
* Store resident's clothing.
* Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.
* Transport residents to new rooms or to the receiving area.
* Assist with loading/unloading residents' to/from vehicles as necessary.
* Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
* Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.
* Report all complaints and grievances made by residents.
* Participate in and receive the nursing report as instructed.
* Follow established policies concerning exposure to blood/body fluids.
* Make beds as instructed.
* Put extra covers on beds as requested.
* Ensure that residents who are unable to call for help are checked frequently.
* Answer resident calls promptly.
