Orchard Post Acute, a skilled nursing facility in Fresno, is seeking a knowledgeable and detail-oriented MDS Nurse (LVN) to join our clinical team.
This full-time position is ideal for a licensed vocational nurse with experience in MDS coordination, PDPM tools, and interdisciplinary care planning.
Position Summary:
The MDS Nurse is responsible for overseeing and coordinating all aspects of the MDS process.
This includes assessing new admissions, scheduling and submitting assessments, tracking quality measures, training new coordinators, and driving care planning and compliance across the interdisciplinary team.
