Linwood Meadows Care Center, a leading skilled nursing facility in Visalia, CA, is seeking a Full-Time Speech Language Pathologist (SLP) to join our compassionate and collaborative rehab team.

Position Summary:
The Speech Language Pathologist is responsible for evaluating and treating residents with speech, language, cognitive-communication, and swallowing disorders.
This role ensures timely, effective, and compassionate treatment aligned with physician orders and facility policies.
Key Responsibilities:
* Evaluate patients within 24 hours or 1 business day of physician referral
* Develop treatment plans and obtain physician approval
* Administer treatments per the approved plan of care
* Supervise SLP Assistants and CFY clinicians in patient care activities
* Communicate effectively with the interdisciplinary team regarding patient status and progress
* Attend and participate in Patient Care Conferences, UR meetings, and staff in-services
* Document evaluations, daily notes, progress notes, recertifications, and discharge summaries per state and payer requirements
* Provide caregiver and staff education for post-discharge maintenance programs
* Ensure compliance with facility policies and the State Practice Act
Qualifications:
* Bachelor's degree in Speech Language Pathology required; Master's or Doctorate preferred
* Current California SLP license in good standing
* Strong proficiency with electronic documentation (Casamba, PointClickCare preferred)
* Excellent communication, clinical reasoning, and organizational skills
* Ability to supervise CFY clinicians and collaborate with restorative staff
Physical & Work Environment Requirements:
* Frequent standing, walking, pushing/pulling, hearing, and speaking
