The Registered Nurse is responsible for the delivery of patient care through nursing process of assessment, planning, implementation, and evaluation.
The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience.
The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Job Requirements:
* Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided.
* One (1) year of home health care experience preferred.
* One-year supervisory/management experience with direct care staff preferred.
* Excellent observation, verbal and written communication skills, problem solving skills, basic math skills;
nursing skills per competency checklist.
* Prolonged or considerable walking or standing.
Able to lift, position or transfer patients.
Able to lift supplies
and equipment.
Considerable reaching, stooping, bending, kneeling or crouching.
Visual acuity and hearing
to perform required nursing skills.

Medical Hill Healthcare is seeking a compassionate and motivated Occupational Therapist (OT) to join our in-house therapy team.
Help residents regain independence, improve daily function, and achieve their rehabilitation goals in a supportive, patient-centered environment.

Responsibilities
* Evaluate residents and develop individualized treatment plans

* Maintain accurate and compliant documentation
* Utilize therapeutic modalities to enhance patient outcomes
Qualifications
* Graduate of an accredited Occupational Therapy program
* Current California Occupational Therapist license (or eligibility for licensure)
* Strong communication and teamwork skills
