The Licensed Practical Nurse is responsible for the delivery of patient care through nursing processes of implementation and evaluation; under the supervision of a registered nurse.
Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Works in accordance with agency philosophy, standards of professional nursing practices, and all federal and state laws and regulations, including HIPAA as applicable.

* Participates in quality assessment and assurance activities including implementation of recommendations and monitoring of results.
* Maintains ongoing communication with other disciplines to evaluate client responses to interventions, suggest alternative approaches when indicated and to revise and update plans of care to reflect progress toward goals.

Key Responsibilities

* Monitor resident condition and report changes promptly
