The service provides person centred support to those with complex mental health needs to develop their skills to live more independently in the community.

What you’ll do


* Creating person centred support plans with our customers, working with our clinicians to develop customers Risk Management Plans.


* Sporting customers with independent living skills such as; budgeting, maximising customers income, cooking, engaging with external services.


* Promoting Home Group’s values within your team.


* Work in close collaboration with; local authority, NHS mental health teams, safeguarding teams and housing option teams.

You have


* Passion to support customer to live their best life, working together with an eye for detail.


* Experience of coordinating and signposting to internal and external services.


* The ability to work on your own initiative, remain calm under pressure and have a resilient approach.


* Recognise every customer is different and you’re able to adopt a flexible, personal approach, listening to customers to understand their needs.

The practical bits


* Where we’re flexible with you, we do need to meet the needs of our customers.

That may mean adapting your start or finishing times on occasion.


* Working to a set rota pattern of day shifts, including weekends and Bank Holidays.


* Able to use technology to update outcome spreadsheets, complete online learning and to collaborate with colleagues or stakeholders.
